Seven steps to support

If you or someone you love would like to receive our support, here’s the process.

1. Reach out

Call us, email us, or complete a referral form, whichever works for you. We’re here to listen, not rush. If it feels like a good fit, we’ll move forward together.

We’ll send you a short form to collect key details about the participant. This helps us start planning the right supports, paperwork, and potential staff match.

Once we’ve reviewed your referral, one of our friendly team members will arrange a home visit or meeting in your preferred location. We’ll take the time to:

  • Talk through your needs, preferences and goals
  • Explain how our services work
  • Discuss what kind of support worker would suit you best
  • Go through any required documentation in a relaxed and respectful way

Behind the scenes, our team begins coordinating your preferred days, times, and finding the right worker for you. We’ll check in regularly and won’t move forward without your input.

Once we’ve found a great match, we’ll organise a meet and greet so you can decide if the connection feels right. There’s no pressure here, it’s all about comfort and trust.

We’ll confirm your first support sessions and start working toward your goals. Your journey, your pace.

After your first few sessions, we’ll follow up to see how things are going and make any changes you need. Your feedback always shapes what happens next.

1. How Do I Apply for NDIS Funding?
  • You’ll need to meet NDIS access requirements based on age, residency, and disability support needs
  • Start by contacting the NDIS directly or visiting www.ndis.gov.au
  • Your GP, allied health professional, or Local Area Coordinator (LAC) can help with your application
  • Once approved, you’ll work with an NDIS planner to set up your plan and goals
  • Registered providers (like SHCS) are approved by the NDIS Quality and Safeguards Commission and must meet strict standards
  • They can work with all types of NDIS plans (NDIA-managed, plan-managed, or self-managed)
  • Unregistered providers can only support plan-managed or self-managed participants, and are not audited by the NDIS
  • Choosing a registered provider gives you extra peace of mind around safety, compliance, and accountability
  • Support Coordination helps you understand and use your NDIS plan
  • A Support Coordinator works with you to find the right services, connect with providers, and manage appointments or changes
  • They help you stay in control of your supports and build your capacity to manage your plan over time
  • It’s about making your NDIS plan work for you, in real life
  • SIL is a type of support for people who live in a shared or individual home and need help with daily living
  • This can include support with cooking, cleaning, personal care, and building independence
  • SIL is usually funded in your NDIS plan under Core Supports or Supported Living
  • At SHCS, SIL isn’t just about living somewhere — it’s about building a life with the right support around you
